Absorbable Suture Materials Explained by Jack
Surgical suture, a medical tool to hold injured tissues together, includes a thread fastened to a needle. Different needle shapes and sizes, and various thread materials are available these days. Non-absorbable and absorbable suture are the two types of sutures available in the market these days.
Absorbable sutures include the catgut (from the sheep's intestine) and the current synthetics such as the polyglycolic acid, polylactic acid, caprolactone and polydioxanone. These absorbable suture materials lose their tensile strength and dissolve by hydrolysis and proteolytic enzymes degradation that could take around ten days to at most eight weeks, based on the materials used.
The manufacturing process of these sutures is similar to that of the strings of the guitar and violin or the strings used in tennis racquets. Presently, gut sutures come from beef or sheep intestine specially prepared for sutures. Absorbable suture materials may be natural (plain gut), treated with chromium salts in order to enhance their constancy in the body (chromic gut) or treated with heat for fast absorption (fast gut).
However, most of these sutures are synthetic fibers, which may come in monofilament or braided threads. They are cheaper, easier to handle, has less tissue reaction and non-toxic. Japanese and European hospitals banned the absorbable gut sutures due to the concerns regarding the mad-cow disease. Although manufacturers certify that, the sources of these sutures are free of this problem and passed the requirements of health agencies.
Patients who cannot come back to the clinic or hospital for suture removal are the ones who benefit from this type of suture. Operation on internal body tissues utilizes this type of suture as well. These absorbable materials keep the tissues together just to allow enough time for the tissues to heal. After a while, these suture materials dissolve without leaving any traces or foreign substances. There are times, though, that the suture materials bring about inflammation leading to the rejection of the surgical suture.
Choosing the right type of suture depends on the type of surgery. They come in many designs to give surgeons several options. What is important to consider is the rate of suture absorption not just along the segment of the thread but most especially at the knot to ensure proper wound closure. Absorbable suture materials ideally need also to have uniformity in size and diameter, pliability for easy handling and securing of the knot, uniformity of tensile strength, sterility and non-toxic (does not have tissue irritants).
The surgeon must remember that increased absorption occur in patients with infection, protein deficiency or fever which may result to a rapid degeneration in the tensile strength of the suture. This problem may also occur in areas full of fluid or wrong handling of the suture prior to use. Sutures work as protection seal and needs to be sterile all the time to prevent serious infections.
Absorbable suture materials have many advantages especially for operations in very delicate parts of the body such as the eyes. The creation of this practical surgical tool contributed to lots of successful one-time surgeries.
